Case Management assists individuals eligible for services from the Division of Developmental Disabilities. Case Managers help find supports and services to meet your personal goals.

Your Case Manager will meet with you and assist with developing a written plan to meet your individual needs and goals. This may include locating needed resources, community and employment services and transitioning from school. You should expect your services to be individualized and provided in a courteous and prompt manner.

Other services a case manager can provide include:

  • advocacy
  • linking to leisure and recreational activities
  • requesting services from local, state, and federal programs
  • employment opportunities

Community Support Services provides Case Management in the following counties:

  • Barry
  • Cedar
  • Dade
  • St. Clair
  • Vernon
